As we age, there are certain patterns of change that are commonly seen in our faces. These changes occur due to a variety of factors including changes in the elasticity of skin, sagging tissues, and loss of facial volume in the form of fat and bone reabsorption in the face.  The face starts to look like a deflated balloon.

Genetics play a major role in how one ages along with one’s daily habits.  Excessive sun exposure and smoking definitely accelerate the aging process. Unfortunately, no one can stop the aging process, but there are many options for rejuvenation of the aging face in order to reclaim a more youthful look and show the world a refreshed, natural looking face.

​​Facelift Surgery - Rhytidectomy

The medical term for a facelift is rhytidectomy.  Many people consider facelift surgery as they age, if they want to project a youthful appearance, an inviting expression, and a well-rested, healthy and refreshed look.  Noninvasive treatments such as injectable fillers, chemical peels and laser treatments only go so far in improving sagging tissues, facial definition and loose and wrinkled skin.  Ultimately, skin tightening procedures through surgery, are necessary to elevate sagging tissue, tighten muscles, improve facial contours and skin appearance.

The term “facelift” frightens many people who are afraid of the stigma of plastic surgery where they have a pulled, distorted, unnatural look. This is not the goal anymore.  Today’s goal in facial plastic surgery is to create a natural refreshed look, so you look like yourself, only years younger.  A critical aspect in performing plastic surgery of the face is for your surgeon not only to address sagging skin but also to address the facial fat volume loss that occurs with aging.  When these 2 issues are addressed simultaneously a more natural look is achieved, not the distorted wind-blown look from “pulled” skin that was prevalent 20 years ago.​

​There are several techniques used in facial plastic surgery.Each approach to facelift surgery is individualized per patient, taking to consideration  their areas of concern and personal goals.  An efficient, effective and minimally invasive procedure is desired along with longevity of results.  Procedures can include a full face lift, neck lift, mid face lift, neck liposuction, mini-face lift and facial fat grafting to restore lost volume gras well as laser resurfacing to improve skin quality.  The goal is to have the entire face look harmonious and well balanced. Facelift surgery is done as an outpatient and is  usually performed under general anesthesia.  Incisions are hidden in  and around the hairline  and around natural ear contours.  The  skin is separated from the fat, supportive structures and muscles below.  Fat may be trimmed or suctioned from around the chin  and neck to improve contour. Sometimes implants are placed to improve facial structure and projection. Loss facial volume is restored to the areas of the face that are deficient, typically the temples, cheeks,  lower eyelid hollows, peri-oral areas and jawline.  Avoidance of over-correction and naturalness is the key.  Very fine sutures are used to close the   skin incisions.

​The goal of facelift surgery is to provide the patient with a more youthful, refreshed appearance that is also age-appropriate.  Surgery will improve skin tone and wrinkles, neckline profile and jowls. Surgery to the neck will help to reduce neck fullness and fatty deposits while tightening the muscles of the neck which restores a more youthful neck contour.

Recovery from a facelift takes about 1-2 weeks.   You will experience some tightness, swelling and bruising after surgery, but most patients are able to resume a public appearance in about 10-14 days.  Because patients continue to age, experience gravitational forces and have weight fluctuations, there is no such thing as a permanent result.  Many factors influence the longevity of the result.  These include patient lifestyle, overall health, skin-care regimen, sun avoidance, diet, exercise, genetics and maintenance regimen.  Results can last 10 years or more, with individual variability depending upon many of the factors mentioned above.

Forehead & Brow Lift Surgery

What is a Brow Lift?

A brow lift is a cosmetic surgical procedure that remarkably rejuvenates the entire face by naturally elevating the upper third of the face and which directly counteracts the gravitational forces that are pulling the brow down, creating an aged and tired look.  In addition, forehead lines and furrows tend to make a person look angry, and the smoothing effect of a brow lift can change your look from unapproachable and tired, to more pleasant, relaxed and inviting.

Types of Brow Lift Surgery

Increasingly, surgeons are using the endoscopic method for brow lift surgery which is much less invasive than previous techniques, yet can achieve an equally desirable result.  With an endoscopic brow lift, the surgeon will make a series of tiny incisions (usually less than ½”) just behind the hairline.  Then using a tiny camera and thin instruments, the muscles and underlying tissues of the forehead are repositioned removing excess fat and tissue to create a naturally more youthful brow.

A temporal or limited incision brow lift involves slightly longer incisions than an endoscopic brow lift and is most commonly performed in conjunction with eyelid surgery.  Incisions about 1 inch long are made behind the hairline, above each temple.  Through this incision the surgeon will lift and reposition the tissues of the eyebrow area.  Then through the incisions made for the upper eyelid surgery, the area between the eyebrows is lifted to smooth out frown lines.

Brow lift surgery is done as an outpatient under general anesthesia or light IV sedation.  The hair is not shaved in order to preserve the hair follicle.  Efficient endoscopic technique is preferred since it minimizes the incision length resulting in less pain and decreased potential for hair loss.  Maximal brow elevation is desired at the time of surgery as some relaxation will occur over the next several weeks.  Brow elevation is never overdone and always looks natural.  Recovery from brow lift surgery takes about one week.

​Facial Implants

What are facial implants?

Facial implants are specially formed solid materials compatible with human tissue designed to enhance or augment the physical structure of your face.  The precise type and size of facial implants best suited for you requires an evaluation of your goals, the features you wish to correct and your surgeon’s judgement.

If you are bothered by a small chin, weak jaw, or lack of facial contour, plastic surgery with facial implants may benefit you.  While any area of your face can be improved with facial implants, the most common sites for facial implants are cheek or malar area and the chin and jaw.

Chin implants can increase the size and projection of a chin that is not in proportion with the forehead and mid-face.   With aging, patients can develop a receding chin, thus correction with a chin implant can restore a youthful profile

Jaw implants increase the width of the lower third or your face. Much like a chin, a weak jaw can be thought of one that is not well defined and distinct from the neck or one that slopes rather than angles from the ear to the chin.  In some cases, both chin and jaw implants can contribute to facial balance.

Cheek implants increase the projection of the cheekbones.  They add volume to areas that may be recessed or flat.

Eyelid Surgery - Blepharoplasty

most people notice when looking at someone is their eyes.

The eyes are one of the most expressive areas of the face. They reveal

our feelings, our openness, our mood and character.  Unfortunately it is

one of the areas of the face  that starts to show aging the earliest.

​As we age, eye appearance changes from gravity, fatty tissue changes, and regular facial expression.  Eyes become droopy, puffy, wrinkled, strained and tired looking.  Patients usually complaint that people tell them they look tired.  People  also develop wrinkles around their eyes also known as crow’s feet or crepiness of the skin of the lower eyelids, heaviness of the upper eyelids, difficulty applying make-up and in some situations, difficulty with vision due to excess skin causing hooding of the eyelids.

Besides the natural aging process, anatomic factors and genetics can predispose individuals to decide to have surgery to improve the appearance of their eyes.  Both young and old patients may complain of bags and dark circles under their eyes. The eyebrows can be naturally low or descend over time with aging and contribute to the appearance of excess skin or fullness involving the upper eyelids.  The cheek fat pad may descend over time as well, which causes lost facial volume and may accentuate a hollowness of the orbital anatomy.   The upper eyelid muscles can become weak, causing a droopy upper eyelid related to muscle function and not excess skin.

Blepharoplasty eyelid surgery is a very common procedure done to reverse signs of aging and to restore a youthful, well-rested look to one’s eyes.  It involves removal of redundant skin and/or fat from the upper and lower eyelids.  In patient’s whose low brows contribute to the upper eyelid fullness, a brow lift procedure may also be indicated.  In patients with dark circles from hollowness, ideally the addition of volume to that area is the best treatment.  This can be done with dermal fillers such as Restylane and Belotero, or your own fat.  In patients with droopy upper eyelids from muscle weakness, upper eyelid muscle repair is often indicated.

Natural looking results are the goal to eyelid surgery.  Surgery needs to be done conservatively preserving your natural eye appearance shape so that you look like yourself, only a younger, more refreshed version.  Preservation of fatty tissue in many cases is desired, to maintain a youthful look and to decrease the risk of volume deficiency and hollowness of the eyes as you continue to age.

Blepharoplasty surgery is performed as an outpatient under general anesthesia or local anesthesia with light sedation.  Incisions are made on the upper eyelid following the natural crease and on the lower eyelid, just below the lash line.  Sometimes, if there is no need to remove skin, a transconjunctival incision is made which is an incision on the inside of the lower eyelid resulting in no visible scars.  Usually skin and extra fat is removed, the lower eyelid is supported and the incisions are closed with very fine sutures.  The incisions on the eyelids heal extremely well and leave almost no visible scar.

Blepharoplasty surgery is a gratifying and straightforward procedure which predictably improves eye appearance and enhances your best feature.  A refined, natural, non-operated look is essential.  While it is normal to experience continued appearance changes from aging, gravity, environment and lifestyle, in the majority of cases the results can last a lifetime.

Ear Surgery - Otoplasty

What is an Otoplasty?

Otoplasty is a surgery to correct abnormalities in the structures of the ears. The main concern is that the ears are too prominent or project out too far from the head.

Earlobe surgery is a type of cosmetic surgery.  Torn or stretched earlobes from earrings, ear piercings or gauges can be repaired to restore a complete and natural looking earlobe.  In addition, saggy and redundant earlobes from aging, gravity or previous surgery, can be reduced to a more normal appearance.

Otoplasty surgery can be performed on children or adults.  For children the best age is at about 5 years, or before the child is about to start school.  This minimizes the social pressure a child can experience when entering school.

Otoplasty surgery is done as an outpatient under general anesthesia.  A small incision is made behind the ear and the ear cartilage is re-contoured.  The ear is then pinned or sutured closed to the scalp to reduce prominence. A compressive dressing is worn for the first week, followed by an elastic headband that is worn for an additional 6 weeks.  Contact sports must be avoided during this time.

Earlobe surgery can be performed in the office under local anesthesia.  Redundant portions of the earlobe are removed, and the earlobe is reshaped and sutured.  Discomfort is minimal and little to no time off work is typical. No exercise is advised for one week.  Sutures are removed usually 7 to 10 days later.

Nose Surgery - Rhinoplasty

An ideal nose is natural looking, proportioned and well balanced with your face.  Rhinoplasty surgery, commonly  known as a “nose job” is one of the most frequently performed cosmetic procedures.  Located in the center of the face, the nose has a dramatic impact on our appearance.  Many times, small refinements to the nose can return balance and harmony to the face, resulting in a profoundly positive effect on self-esteem and confidence.

Typically the incisions for a rhinoplasty are well hidden with the only outside incision being on the undersurface on the nose, between the nostrils.  Operation time for a rhinoplasty can take anywhere from 2-4 hours depending upon the extent of work that needs to be done. Sometimes, bone graft or cartilage may be obtained from another part of your body to use in the nasal reshaping or reconstruction.

Neck Lift - Platsymaplasty

Of all cosmetic procedures, one that can make the most significant improvement to a patient’s profile is a neck lift.  This surgery is ideal for patients that have had a significant amount of weight loss as well as for those that have loose, sagging skin in the neck area due to aging and the effects of gravity.  Neck lift surgery removes excess fat and sagging skin to produce a more tightened, slimmer profile. It removes what some refer to as a “turkey neck” or “wattle”.   A neck lift not only tightens your skin but it also restores definition to your jawline.  Although there are non-surgical options for improving your neck’s tone, a surgical lift, often combined with liposuction, can produce dramatic, predictable and long lasting results.

For many men and women, neck lift surgery can take years off of their appearance.  A neck lift is really the only solution that offers a predictable result  when it comes to redefining an individual’s neck to create a strong contour and youthful jawline. A neck lift removes hanging skin and reshapes the neck.  During the procedure,   an incision may be placed under the chin and sometimes behind or in front of the  ears as well.  For patients with visible banding of the neck a platsymaplasty may be needed as well. Banding in the neck results from laxity and separation of the sheet like platysma muscles.  During neck lift surgery, the muscles are surgically  repaired and tightened with sutures to correct the muscle diastasis. Excess fat can be directly excised or removed with liposuction. Subplatysmal fat or deep neck fat is not amenable to liposuction and must be directly excised.


​A full neck lift involves liposuction, repair of the platysma muscles and repositioning of the skin over the repaired muscle. Excess skin is removed through well -hidden incisions.  Sometimes a neck lift may be combined with a facelift or even a chin implant. It is common for these additional procedures to be combined to achieve the patient’s desired aesthetic outcome and goals.  During your consultation, your surgeon will discuss what options would be best for you.  Many times patients combine procedures in order to minimize downtime and maximize results.

The procedure is performed under general anesthesia as an outpatient.  The duration of the surgery usually takes around 2 hours, but it could be longer if other procedures are combined with the neck lift surgery. Bruising and swelling will be visible after surgery, but this usually subsides in just a few days.  Initially patients will feel some tightness and numbness.  Once the neck adjusts to its new position, patients will notice a beautiful youthful neck contour and a more comfortable feeling with regards to the “tightness”.  Most patients are able to resume their work and social activities after 2 weeks. No strenuous exercise is permitted for 6 weeks.  The complete healing process for any facial surgery takes about 6 to 12 months, with improvements occurring over time.
