Most people are not fortunate enough to be born with perfect body proportions or contours.  In addition, unhealthy lifestyle choices, pregnancy and lack of exercise can lead to diet resistant bulges and unwanted fat deposits. Skin can stretch; fatty deposits can accumulate as a result of aging, normal weight fluctuations, hormonal changes and gravity.  Following a healthy diet with weight loss and exercise can help to some degree, but body contouring can help patients to reach goals quicker and that are sometimes impossible to achieve otherwise.

Liposuction or Bodysculpture is an excellent way to remove fat, but it does not tighten skin.  The best candidates for liposuction generally have good skin tone, meaning smooth elastic skin with minimal stretch marks, minimal cellulite, wrinkling or looseness.

Our surgeons perform the tumescent liposuction technique.  A formula consisting of normal saline, lidocaine, epinephrine and sodium bicarbonate is injected into the subcutaneous fat. This solution does 3 things - #1 and most important it anesthetizes the areas where we will be removing the fat, # 2 it emulsifies the fatty tissues meaning that the fat becomes liquefied making it much easier to extract and #3 the epinephrine in the solution produces vasoconstriction of the blood vessels that supply the fat thereby significantly reducing any bleeding.  Tiny incisions are made in which an instrument called a cannula is introduced and with a gentle suction technique the fatty tissue is removed and the body contoured and sculpted in the process.  Compression dressings are applied and the patient is placed in a post-surgical compression garment which helps to redrape the skin and reduce swelling.

The surgical procedure takes approximately 2-3 hours depending upon how many areas are being treated and the volume of fat being removed.  The fat cells are permanently removed and even if a patient would gain weight after the procedure, normally the treated areas maintain a better body contour. Common areas that we treat include the abdomen, back, flanks, waist, male chest, thighs, arms and chin and neck area.  Most patients are able to return to work within 2-3 days although everybody heals different and some patients prefer taking more time off after the procedure.  All patients are encouraged to maintain their results by incorporating lifestyle changes including healthier dietary habits and light exercise.  Although improvements in your body contour are seen relatively quickly after your surgery, it generally takes a good 3-4 months for all the swelling and inflammation to fully subside and for you to appreciate your new sculpted shape.

Brazilian Butt Augmentation

The Brazilian Butt Augmentation also known as BBL for Brazilian Butt Lift is  a surgical procedure that is commonly done in conjunction with liposuction which results in a more rounded, youthful and sensual buttock appearance. The procedure uses your own fat, so it is the most natural way to augment your buttocks.  Over the last  several years the buttocks have received more press coverage that ever before with some prominent celebrities showing off  their assets.  People of all ages and body types are having the Brazilian Butt  Lift Procedure.

The Brazilian Butt lift can address problems such as flatness of the buttock region,  sagging buttocks, depressed areas of the buttocks, asymmetrical buttocks and small disproportionate  buttocks. Florida Law limits the amount of  fat that a surgeon can  transplant at one time for safety   reasons. With the BBL procedure, fat is removed with liposuction and the lower back and flanks are suctioned to create a more sculpted appearance.  The fat is then processed in a special manner in order to prepare the fat to be injected back into the buttocks. The procedure is typically designed to fill the upper quadrant of the buttocks so that the buttocks appear lifted and perkier with more projection. Each patient is assessed individually as some patients may actually require the fat to be placed in a different position to correct their particular problem.

Following the surgical procedure, you may not sit directly on your buttocks for 3 weeks after your procedure.  This allows for the best take of the fat graft in the buttocks and ensures the most long lasting result. If you do not follow these instructions, you will not achieve the greatest survival of the fat graft.

Tummy Tuck- Abdominoplasty

The medical term for a tummy tuck is Abdominoplasty. Tummy tucks are one of the most popular cosmetic surgery procedures that patients elect to have.  The reasons are varied.  Many women after having babies end up with bulging bellies, loose skin and significant stretch marks and nothing they do seems to help. Other woman who have had drastic weight loss or fluctuating weight throughout their life, develop very poor skin tone with flaccidity of the abdominal skin.

If you want a flat abdomen and want to look good in form fitting clothes and s bikini again, then a tummy tuck is the ultimate way to tighten you abdomen and narrow your waistline.  Men also can benefit from a tummy tuck if  they have a lot of loose skin from weight loss.  Being overweight is not the only reason people experience abdominal bulging.  There can be loose and sagging skin with stretch marks, or previous scars from surgery or c -sections. The belly button may look wrinkly and unattractive. No amount of diet or

exercise will help in situations like this.

The procedure is usually done as an outpatient under general anesthesia.  The operative time can range anywhere from 2 to 5 hours depending upon the extent of the work that needs to be done.  An incision is made in the lower abdomen generally from hip to hip.  The length of the scar depends upon the amount of excess skin that is to be removed.  Old C-section scars will be removed.  Patients with small amounts of excess skin - like 1 - 2 inches, may be a candidate for what is known as a short scar abdominoplasty or mini tummy tuck. Extended abdominoplasty is when the incision is carried posteriorly over the flanks and hips to reduce excess skin in those areas.  Panniculectomy refers to the complete excision of overhanging skin and fat in an obese patient.  In this case the overhanging skin and fat is called a pannus.

If your pubic area is chubby or saggy, a simultaneous pubic lift can be

done in conjunction with the tummy tuck.  Some patients require the

underlying muscles to be repaired at the time of their abdominoplasty

surgery in addition to the excess skin removal.  If you have seen patients

who after having tummy tuck surgery still have a  protuberant abdomen,  it

usually means that the surgeon did not take the extra step to incorporate the

muscle repair at the time the tummy tuck was performed.   Sometimes

small volume liposuction is performed at the same time of the tummy tuck

Recovery from tummy tuck surgery takes approximately 2 weeks. It is important that you have someone at home with you to help you for several days after surgery. You will use an abdominal binder for support after surgery and it is important to walk with a slightly hip-flexed position to keep tension off the incision.  You will need to maintain a 45 degree angle when you sleep, therefore if you are able to sleep in a recliner for the first week after surgery, this helps to maintain the hip flexed position in order to reduce tension on the abdominal incision.  You will be uncomfortable for the first week after surgery. Usually by the second week most patients are moving around better and feeling less discomfort.  Most people typically go back to their normal activities of daily living and non-physical work 2-3 weeks after surgery.  No strenuous physical activity is permitted for 6 weeks including no heavy lifting, aerobic exercise or straining of abdominal muscles.  After 6 weeks gradual resumption of physical activities is permitted but only as tolerated.  Patients are encouraged to maintain a healthy diet as weight gain after the procedure can negatively affect the results.

Patients after tummy tuck surgery experience life-changing improvement in their confidence, self-esteem and quality of life. Mommy Makeover

What is a Mommy Makeover?

After Pregnancy, it is not uncommon for many women to experience a lot of changes in their body that they do not see as positive. Because of abdominal stretching with the pregnancy, you may have developed excess and sagging skin, cellulite, stretch marks and even a change in your abdominal musculature due to tearing of the muscles and changes that occurred while pregnant and giving birth. As a result of pregnancy and nursing, breasts may have developed a deflated appearance with sagging skin, stretch marks and stretched areolas.  You may also have developed excess fat in areas that you never had fat, due to the hormonal changes that occur with pregnancy.  Many women after pregnancy have a hard time with diet and exercise restoring their pre-baby

A mommy makeover is typically a combination of procedures designed to restore your body to its pre-pregnancy shape. The procedures included can vary from individual to individual depending upon each patient’s specific need, but this is something that can be discussed with your plastic surgeon at the time of your consultation.  The procedures are usually designed to eliminate excess fat and stretch marks, fill and lift your breasts and restore your abdomen to what it was like before your pregnancy.  A mommy makeover usually combines multiple procedures in one operative session in order to minimize downtime and allow for one recovery which is essential for busy moms.

Procedures that can be included in Mommy Makeover may include any of the following in order to restore your pre-pregnancy shape as best as possible.  This should be undertaken when you are done with your childbearing years and breastfeeding, and are ready to get your body back to what it was before by utilizing a combination of the following procedures:

Breast Augmentation - by placing breast implants we can restore your breast’s fullness, size and perkiness.

Breast Lift -breasts often become saggy and deflated after pregnancy and nursing  so a breast lift procedure also known as a mastopexy will bring your breasts back up to the proper position where they will look in proportion to the rest of your body.

Breast reduction - this is usually done for women with large, saggy breasts.

Tummy tuck - also known as abdominoplasty - In this procedure, excess portions of abdominal skin, fat, stretch marks are removed from your belly area.  In addition, many patients require the underlying abdominal musculature to be repaired by placing sutures in the muscle wall to repair and restore the rectus abdominis muscle to its pre-pregnancy state.

Liposuction - By utilizing the micro-cannula technique, we can shape and sculpt your body and expertly remove fat pockets from your abdomen, waist, thighs, hips, back, bra line, arms, chin and neck

In order to be a good candidate for a Mommy Makeover, you should be done having children and nursing, as another pregnancy can reverse the effects of the procedure performed. It is imperative also to be at a healthy weight.  If you are slightly overweight (15 to 30 pounds) you may still be a good candidate provided you are in good health.  Mommy Makeover procedure is great for women who do not feel confident with their bodies anymore and want to look great again in a swimsuit or bikini.

The procedure is done under general anesthesia and can typically take anywhere from 4-6 hours depending upon the amount of work to be done.  Following surgery, you will need someone to help take care of you for 3-4 days after your surgery.  You will not be able to drive or lift small children during this time. You will be uncomfortable for the first 2 weeks, but usually by the 2nd week patients are moving around better and are able to resume some of their non-strenuous daily activities.  Usually no strenuous exercise is permitted until 6 weeks post-op.  Most patients are able to go back to work after 2 weeks.

A Mommy Makeover is a huge self-esteem boost.  The confidence you get from having beautiful breasts and a flat tummy again will have a positive effect on your life and your attitude.  If you are a busy mom and have breasts and a body that make you feel unattractive and depressed, take time for yourself to

improve what diet and exercise cannot.  The small amount of downtime is worth it and you will have your new look for the rest of your life.  Call  The Best U  Now  to schedule your consultation and take the first step to reclaiming the body you used to have.

Arm Lift - Brachioplasty

Many people develop loose skin along the upper inner arm from aging, fluctuations in weight and gravity. Individuals who have had major weight loss usually develop extreme excess skin in this area which can interfere with daily life and make it difficult to find clothes to fit.

Arm lift surgery otherwise known as a brachioplasty removes and tightens the skin of the upper inner arm.  The trade- off is a potentially visible scar along the full course of the upper inner arm. For patients with severe excess skin, it is worth the trade-off.  Patients that have only minimal excess skin may be able to have a short scar brachioplasty where the scar is more hidden into the armpit or axillary fold.  Small tucks in this area provide subtle improvement, but may be effective for those with only small amounts of excess skin.

Some patients with good skin tone and disproportionate amounts of fatty tissue of the upper arms may benefit from liposuction alone to decrease fullness of the upper arm without any visible scarring.  In general, patients with poor skin tone and not a significant amount of fat of their upper arms are better off with a non-surgical approach, using diet and exercise to improve their upper arm contour.

In patients with moderate to severe fatty deposits along the upper arm, a 2-stage procedure may be the best approach.  The first stage would be to do aggressive liposuction of the upper arms to reduce the amount of bulky fatty tissue.   The 2nd stage would be the brachioplasty in which the excess skin would be excised and tightened.  Aggressive liposuction at the time of upper arm skin tightening is not recommended due to swelling and circulation changes that may hinder proper healing of the incisions.

Brachioplasty surgery is usually performed as an outpatient under general anesthesia.  Surgery can take 2-4 hours depending upon the extent of work that needs to be done.  Arms will be bandaged in ace wraps for about one week and then the patient will use a type of a sleeve compression garment for several weeks. Most patients take 1-2 weeks off of work.  No heavy lifting or excessive exercise for about 1 month.

Arm lift surgery is an effective way to treat excess skin of the upper arms. It has a high rate of satisfaction for the right candidate.  Patients need to understand that there is a trade-off - the excess skin will be removed, but in its place there will be a long linear scar.  Patients must have realistic expectations understanding that results vary from patient to patient.  Currently there is no other technology that effectively tightens the skin of the upper arm as dramatically as a brachioplasty.   To find out if an arm lift procedure may be the right surgery for you, please call  The Best U  Now  to schedule a consultation.

Thigh Lift Surgery

A thigh lift is a cosmetic surgery procedure designed to tighten the skin of the thigh and to reduce sagging of the inner or outer thighs. The procedure is commonly performed on people who have undergone massive weight loss, but it is also performed on people who have not experienced weight loss.  Saggy inner thighs can also be caused by aging, genetics, lack of exercise and people that have had fluctuations in their weight.  Sometimes patients that have undergone aggressive liposuction to their thighs can develop sagging skin due to poor skin retraction after their procedure.

The inner thigh lift has gained a lot of popularity in recent years due to more people undergoing bariatric surgery for weight loss.   Many people although happy with the fact that they lost weight, are dissatisfied with the hanging skin of their inner thighs that results from significant weight loss.  The outer thighs are not usually affected as much by the factors mentioned above, as much as the inner thighs are.  Also the outer thigh skin tends to have more elasticity and “snap back” effect than the inner thighs.   Therefore, inner thigh surgery is much more popular and more commonly performed.  The inner thigh lift surgery may be combined with liposuction at the same time or for someone with excessive fatty deposits on their thighs, a 2 stage procedure may be performed in which aggressive liposuction in done first followed in a few months by a thigh lift surgery.

To address issues with the inner thighs, there are usually 3 types of procedures that are performed:

1)    Liposuction alone- this is reserved for people that have excellent

skin tone and muscle tone.

2)   Combined liposuction with an inner thigh lift - this is for people that have

excess skin and fat

mostly localized to the upper aspect of the inner thigh.

3)   Extended inner thigh lift - this is many times

combined with liposuction as well and is for people that have excess fat and

poor skin quality extending from the inner thigh to the knee. An outer thigh lift

can be performed for those patients that have excess skin and fatty deposits

that hang below the hip area.  Sometimes this can be combined with a

lower  body lift for the best outcome

Thigh lift surgery is performed under general anesthesia.  Recovery general takes 2-3 weeks.  Compression garments are worn for the first several weeks to reduce swelling and minimize the risk of the development of a seroma.  Activity must be kept to a minimum for the first two weeks to reduce the tension on the incisions. No sitting is permitted for the first 2 weeks and no driving for 3 weeks.  Most patients are able to resume their normal activities in about 4-6 weeks.  No strenuous exercise is permitted for 6-8 weeks.   This surgery has a higher complication rate than many of the other types of surgeries that we perform for a number of reasons.  First of all, many patients find it difficult to adhere to the strict post-op regimen but it is essential for reduced complications. Also this area is more prone to infection therefore it is critical to follow excellent body hygiene after your surgery.
